Gladstone Industry Medical Pass

The Gladstone Industry Medical Pass (formerly known as “Medipass” or “3 Site Pass”) is a medical and functional capacity assessment that allows a worker to access the following sites in Gladstone, Queensland:

Jobfit is an approved medical provider for the Gladstone industry, which allows us to issue the Gladstone Industry Medical Pass.

MEDICAL ASSESSMENT

Workers are required to complete an online medical history questionnaire prior to their appointment, which takes approximately 15 minutes.

The appointment includes a review of relevant medical history, vision, audiometry, spirometry, functional capacity evaluation and doctor review.

The examination can be conducted at any Jobfit centre, and the site access pass is issued after the assessment has been successfully completed. Please note the physical card can only be collected from Jobfit Gladstone.

GLADSTONE INDUSTRY MEDICAL PASSES

There are two kinds of Gladstone Industry Medical Pass:

  • Blue Medical Pass, and
  • Yellow Medical Pass.

Both Medical Passes are valid for 2 years.

SITE PASS

A physical site pass (card) can only be collected from Jobfit Gladstone located at 164B Goondoon Street, Gladstone QLD 4680. An appointment is not required to collect the card. If your medical examination is conducted at Jobfit Gladstone and no further follow up is required, the card will be available for your collection following your appointment.

In addition, you must pay online before we can confirm your appointment. You will need to pay the required fee via the secure payment gateway after you submit your request.

BLUE MEDICAL PASS

A Blue Medical Pass covers all sites, except for specific areas of BSL that require workers to wear a respirator. If a candidate needs to access these areas, they will need a Yellow Pass which has the extra testing component of pre-ventolin and post-ventolin spirometry.

A Blue Medical Pass includes:

  • A comprehensive questionnaire
  • A thorough medical check including height, weight, BMI, BP, pulse etc.
  • Visual acuity (distant and near)
  • Colour vision (Ishihara) and further assessment if the worker fails Ishihara
  • Audiometry
  • Spirometry
  • Drug and alcohol test
  • Functional capacity evaluation
  • A doctor review

Blue Medical Pass appointments take approximately 2 hours. The price of a Gladstone Industry Blue Medical Pass conducted at a Jobfit centre is $653.40 (incl GST). In addition, a credit card surcharge is payable.

    Jobfit performs all drug and alcohol testing according to the relevant Australian & New Zealand Standard (urinary drug testing to Australian Standard AS/NZS 4308:2008 and oral fluid drug testing to Australian Standard AS/NZS 4760:2019). In the event that an instant drug test or laboratory drug test is non-negative, it is mandatory under the Australian Standards for the sample to undergo further confirmation testing within a NATA accredited laboratory before a final result is issued. This further confirmation testing will incur an additional cost, and is not included in the cost of the medical. Payment of the additional fee will be required before the medical results can be released.

    You must pay for the appointment at the time you request an appointment. Once we have received your payment, your appointment will be confirmed via SMS within 48 hours. Please have your credit card or debit card ready to make payment.

    Yellow Medical Pass

    A Yellow Medical Pass includes:

    • A comprehensive questionnaire
    • A thorough medical check including height, weight, BMI, BP, pulse etc.
    • Urinalysis
    • Visual acuity (distant and near)
    • Colour vision (Ishihara) and further assessment if the worker fails Ishihara
    • Audiometry
    • Spirometry (pre and post Ventolin testing)
    • Drug and alcohol test
    • Urine Salbutamol test
    • Functional capacity evaluation
    • A doctor review

    Yellow Medical Pass appointments take approximately 2.5 hours. The price of a Gladstone Industry Yellow Medical Pass conducted at a Jobfit centre is $764.50 (incl GST). In addition, a credit card surcharge is payable.

      Yellow Medical Passes require spirometry testing every 6 months to remain valid. This is known as a Gladstone Industry Medical Pass Update.

      A Yellow Medical Pass Update includes:

      • Spirometry
      • A doctor review

      Yellow Medical Pass Update appointments take approximately 30 minutes. The price of a Gladstone Industry Yellow Medical Pass Update conducted at a Jobfit centre is $190.30 (incl GST). In addition, a credit card surcharge is payable.

        UPGRADE FROM BLUE PASS TO YELLOW PASS

        A current Blue Pass can be upgraded to a Yellow Pass at any time with the addition of pre and post ventolin spirometry testing. This is known as a Gladstone Industry Medical Pass Upgrade.

        A Yellow Medical Pass Upgrade includes:

        • Spirometry (pre and post Ventolin testing)
        • Urine Salbutamol test
        • A doctor review

        Yellow Medical Pass Upgrade appointments take approximately 45 minutes. The price of a Gladstone Industry Yellow Medical Pass Upgrade conducted at a Jobfit centre is $324.50 (incl GST). In addition, a credit card surcharge is payable.
